To reset your My Health App password:

  • Select the "Forgot Password" link on the login page
  • Enter your user name and select "Next"
  • Select how you want to receive your password reset link - email or SMS. If we only have your email, or only our phone number, then it will be selected automatically. 
  • You will receive an email or SMS with the link to reset your password. If you don't see an email, check your spam folder. 
  • Follow the link to the password reset page. 
  • Enter your new password.
  • Confirm your new password.
  • Once you tap "Submit" your password will be reset. 
  • Go back to your login page, and log in using your new password.
